Most webpages have the coding for the small square icon in the bookmark tool bar. Some do not. How do I add an icon for a webpage that does not have an icon assigned?

Those icons are called favicons. Unfortunately, there is no builtin Firefox feature to customize or change the favicon, since the favicon is determined by a line in the website's HTML code.

There are many Firefox addons that improve your browsing experience and allow you to make certain customization to Firefox. You can browse the Firefox addons here.

The only addon that I was able to find was the Bookmarks Favicon Images addon, but it looks like there are a few restrictions and you are only permitted to pick an icon from the addon's list, not a custom image.
