Plugin and notification "Allow and Remember" choice does not remember.
This happens for notifications for my web-mail as well as Adobe plugin allowance for sites I often visit. Every time I start a new session of Firefox it asks me again, even though I always click "Allow and Remember". Very annoying, but I assume this is not a common problem since not much comes up when I search for this problem.

Your System Details list shows that you have a user.js file in the profile folder to initialize prefs each time Firefox starts. The user.js file will only be present if you or other software has created this file and normally won't be present.
You can check its content with a text editor (right-click: "Open with"; do not double-click). The user.js file is read each time Firefox is started and initializes preferences to the value specified in this file, so preferences set via user.js can only be changed temporarily for the current session.
You can delete the user.js file if you didn't create this file yourself.

The most common reason for this would be the setting to clear history at shutdown, if the "Settings" selections include "Site Preferences". To check on that:
"3-bar" menu button (or Tools menu) > Options
In the left column, click Privacy, then on the right side, make sure it says "Firefox will: Use custom settings for history". If there is a checkmark for "Clear history when Firefox closes" click the Settings button to the right and make sure "Site Preferences" is NOT selected.
If that is not the issue, hmm, are other site-specific settings remembered, such as permission to show pop-up windows?
Thanks for your reply. "Clear history when Firefox closes" is checked, but then the ONLY checkbox checked in that settings dialog is "Cache". I am not too sure about pop-up windows... I don't run into that too often, and probably don't choose to remember that setting.

Thank you. That file was created in March of 2013 and only had one line in it:
pref("extensions.toolbar.mindspark._4zMembers_.defaults.exist","true");
Deleting it did allow Firefox to remember after a restart! Thanks again!
You should remove/uninstall the extension that has added this line.
You can check for recently installed suspicious or unknown extensions.
