I've got menu, navigation and add-on bars showing, but there's no search field. When I open Customize (toolbar), it appears to the right of the address field, where I expect it, but when I close Customize, it disappears. Thanks,

Try to click the "Restore Default Set" button in the customize window to see if that brings back the search bar.

Restoring didn't return the search field. I reinstalled 4.0.1 on top of the current version, but that didn't do it either. But...

I installed the AwesomeBar HD extension yesterday (http://www.downloadcrew.com/article/22671-awesomebar_hd), and it struck me that might be the cause, although nothing in the write ups said it combined the search and address fields into one long one. It took some work to find the extension, since there wasn't any evidence of it in the Extensions list. Rather, I had to dig deep in the Profiles subfolder g9zrxnk6.default, which seems strange for an extension supposedly developed by Mozilla staff. Deleting that extension file returned the search field. Problem solved (I didn't like AB HD)
