how do I remove an address from Bookmarks & Bookmark toolbar?

I need to delete an address that is not used anymore on my toolbar and also in my bookmarks. I cannot find a way to delete them. Please advise.

You can open that site in Firefox via the location bar to get a fresh history entry.
Open the History in the side bar (Cmd+H) and locate that entry and use "Forget About This Site" in the right-click context menu to remove all references to it.
