All downloads are shown as failed yet are still downloading.
Whenever I download a file and go into Downloads it shows the file as failed, but the Firefox icon shows a download progress bar and the file will complete if left alone. The real issue that I'm having is that I have no idea how fast a file is downloading and I cannot estimate when it will finish.
I'm tried reinstalling Firefox and disabling all add-ons, but neither of these worked.

Thanks, I think that it was the theme I was using. It was actually both, on the toolbar and in the Downloads folder.

Do you mean the Downloads button on the Navigation Toolbar or the Downloads folder in the Library (Show All Downloads)?
Start Firefox in Safe Mode to check if one of the extensions (Firefox/Firefox/Tools > Add-ons > Extensions) or if hardware acceleration is causing the problem (switch to the DEFAULT theme: Firefox/Firefox/Tools > Add-ons > Appearance).
- Do NOT click the Reset button on the Safe Mode start window.

Can you tell us which theme caused your problem?
Does the theme show as compatible on the Add-ons page?
I have the same issue. Downloads History shows failed, yet the file still downloads.
Running on Ubuntu. Using stock, standard Firefox with Default theme. Other than the default Ubuntu add-ons, the only extra one I am using is Firebug.
The progress bar on the Unity launcher shows progress, it is just the Downloads history that shows it failed.
