The tab was in a different place before. How do I move it back?

Right click the "+" (new tab) button, or a blank area after the address box and uncheck Tabs on Top.
The refresh icon is now at the end of the address box. If you want to move it, use
View > Toolbars > Customize
it will split off from the address box and you can drag it somewhere else.
If you see the orange Firefox button instead of the classic menu bar, tap the Alt key or press F10 to display the menu bar.
