This is repeatable.. whenever I open a website from a bookmark, and then close the website tab, I get a new bookmark created in the same location as the original, with the same name as the original.

Are you using Sync to sync bookmarks in case this makes a difference?

Start Firefox in Safe Mode {web link} by holding down the <Shift> (Mac=Options) key, and then starting Firefox.

A small dialog should appear. Click Start In Safe Mode (not Refresh). Is the problem still there?

I was suspicious after posting that it might be something to do with iCloud... syncing/backing up Firefox bookmarks.. not something I set up on purpose, but something I did STOP on purpose, as I already backup bookmarks in a different way.. and after stopping, problem solved... I am not a fan of cloud backup, and don't use my phone as much more than a phone/text communication device, and do not browse or email from it, so icloud IS useful for phone backup, but not computer stuff for me..
